CONSENT TO VIRTUAL CARE:  READ TO PATIENT OR SDM

Having visits by video/phone helps to reduce physical contact and protect our patients against COVID-19 and other respiratory illnesses. It is important to understand that during this visit, the doctor may decide that an in-person appointment is required for a clinical examination or may direct the client to visit the nearest hospital for urgent care.

A video visit is one where the client, community partner and doctor communicate using Zoom.  Zoom is easy to use, secure and privacy protective.  Although Zoom is a trusted tool, video visits are not risk free due to the nature of cybersecurity. 

Privacy is one of our top priorities.  We want to ensure you that video or phone visits are made in a private protective setting, where no one but those providing care to the client are present.
